Carotid Endarectomy with Modified Eversion and Mini-Patch

ABSTRACT

Carotid Endarterectomy is generally offered for the treatment of symptomatic or high grade asymptomatic carotid stenosis. While most Vascular surgeons perform the procedure using the standard Linear endarterectomy  incision with or without patching, the eversion technique is a useful alternative.  We report our experience using a modified eversion plus mini patch technique that we find quite useful in the setting of markedly high distal internal carotid stenosis and in marked kinking of the internal carotid artery causing severe clinical symptoms of Transient Ischemic Attack (TIA) or major Stroke.
The procedure offers a mini-incision and allowed for careful evaluation of the distal portion of the internal carotid  to perform thorough endarterectomy to the true transition point and utilization of a mini-patch option if necessary to prevent restenosis.  We believe this treatment option should be in the armamentarium of the Vascular Surgeon.
